Is it okay not to go for surgery for a mild varicocele in my left scrotum?

You risk the inability to have children...maybe. That is the biggest problem with a scrotal varicocele, as you already know. However, it also depends on what your sperm count is. Have you done one already? Sometimes the surgery doesn't improve your semen quality. What has your doctor said? Are you currently in pain? That seems to be the reasonmost men go through with the surgery.
Best of luck to you.If it's not hurting and just in one side it's probably fine to leave it alone. Just the one side means that the other one should be working fine. I've had mine for 40 years with only mild discomfort now and then. They've got less troublesome as I got older.
