Varicocele surgery is fairly simple and almost painless.Epididymitus, or maybe something else like idiopathic varicocele? Not satisfied with phisicians answer.?

Man, I feel your pain, I has Epididymitus and it was the most painful thing ever. All my symptoms were the same as yours. My course of antibiotics cured it though.
A couple of thoughts...

Have you had an ultrasound done on your testes to make sure there is no torsion? This was the first thing my doctor did.

Have you and your wife been tested for Chlamydia or other sexually transmitted diseases? Some of these can cause symptoms identical to Epididymitus. This would explain why it keeps coming back as your wife can be passing it back to you while having no symptoms herself at all.
